Graduation Status, Grades, Final Transcript, Attendance, Immunization Record*. We are not able to guarantee that your immunization record will be part of your record. If it is, we will include it with your Official Transcript.
Verification Letter:
$15 per letter
An official form letter that states Student Name, School and date of Graduation.
Student Record:
$15 per Complete Student Record
Graduation Date, Final Transcript, Attendance, Drivers Ed Certificate if applicable, Immunization Record*. * Immunization records may not be included in your file.
Record:
$15 per Immunization
* Colleges and Universities usually require the State Certification of Immunization form listing your immunization history. We can’t guarantee that this version will be in your file. We can only include the version of your shot record that is contained in your file. Search fees are nonrefundable. To ensure you receive the correct document, we recommend contacting your local health department or asking your current healthcare provider to access it through the North Carolina Immunization Registry.
